WHS Athletic Participation Requirements

Dear Parent/Guardian,

GUIDELINES FOR COMPLETING PAPERWORK FOR All SPORTS:
* A sports physical (NJDOE Annual Athletic Pre-Participation Physical
Examination Form, N.J.A.C.6A:16-2.2(h))MUST HAVE ALL AREAS of parts A & B COMPLETELY filled out and on the School Nurses's desk by the FIRST WEEK IN AUGUST.
* IF ANY PART OF THE FORM IS INCOMPLETE (parts A and/or B) IT WILL BE RETURNED TO PARENT/GUARDIAN TO HAVE PHYSICIAN AND/OR PARENT/GUARDIAN TO COMPLETE.
* EVERY private sport physical submitted to the school nurse, MUST be
REVIEWED & CLEARED by the SCHOOL PHYSICIAN, according to N.J.A.C.
6a:16-2.2.
* According to the WHS Policy, ALL Pre-Participation Athletic Physical Examinations need to be done before August 1st. If insurance is a problem, student athletes can call for an appointment (it is a free athletic physical exam) with WHS Team Physician Dr. Jesus Alfonso, 7907 Kennedy Blvd., North Bergen, NJ 07047, 201.330.8747.
* In addition to the sport physical, each student will need the NJSIAA Steroid Testing Policy, Consent for Random Testing signed and dated by the student and the Parent/Guardian. Also, the Parent/Guardian and Student will need to sign and date the NJSIAA Concussion Policy Acknowledgement Form.

PLEASE CHECK THE SPORTS TEAMS STARTING DATES, to make sure your child's sports physical is completely filled out, dated, signed and given into the School
Nurse's Office by said date. If not they will NOT be able to start with their team mates. All regulations and policies need to be followed so the Weehawken Board of Education and our Health Office is in compliance, and most important to protect the health and care of all our student athletes.
